Deploy a form

  1. Open Form Builder.
  2. Add fields mapped to contact columns/custom fields.
  3. Copy embed code or hosted URL.
  4. Publish on website.
  5. Submit test lead; confirm contact in CRM.

Connect automation

  1. Create Smart Flow triggered on form submit (see Trigger Events).
  2. Add actions: assign owner, send WA template, create task.
  3. Activate; re-test submission.
  1. Calendar Links → create link tied to appointment type.
  2. Share in email signature or SMS.
  3. Booking creates calendar event + contact activity.

Extended guidance

UTM-tagged Link Shortener URLs make ad spend visible in Lead Sources reports. QR codes on print should point to mobile-friendly forms—test iOS and Android scans. Deactivate old forms after events to prevent stale leads entering Smart Flows.
